Long-term care best practices toolkit
The LTC toolkit is designed to offer point-of-care staff, nurses, educators and leaders access to the best available evidence-based resources and tools. It supports the use of best practice guidelines (BPG), program development, implementation and evaluation to enhance the quality of resident care and create a healthy work environment (HWE).

Best practice guidelines
Preventing Falls and Reducing Injury from Falls
This guideline focuses on the prevention of falls and fall injuries in all adults at risk for falls – including those living in the community – who are receiving care from nurses or other health-care providers.
Transitions in Care and Services
This guideline provides evidence-based recommendations for nurses and members of the interprofessional team, organizations and the health system. These recommendations support safe and effective transitions in care for pediatric and adult persons and their support network.
A Palliative Approach to Care in the last 12 Months of life
This guideline provides evidence-based recommendations to nurses and the interprofessional health team who support adults experiencing the last 12 months of a progressive life-limiting illness, and their families and caregivers.

A gap analysis allows teams to focus on what areas need change, and develop a plan for implementing it. We have gap analyses for dozens of issues related to resident care and supporting healthy work environments.
Advanced Clinical Practice Fellowships
The Advanced Clinical Practice Fellowship program provides opportunities for RNs and NPs to develop their skills, knowledge and expertise. How? By advancing projects that will meet a need or gap in service within their organizations, with support from mentors, from the organization that employs them – and from RNAO.
