A Person- and Family-Centred Care Virtual Community of Practice

The RNAO Long-Term Care Best Practices Program is offering a community of practice (CoP) exclusive to Ontario long-term care (LTC) homes to implement the Person- and Family-Centred Care (PFCC) best practice guideline (BPG) practice recommendations through a collaborative peer supported process.

PFCC is now a ‘hot’ topic in Ontario LTC homes, especially with its inclusion in the new Fixing LTC Act (2022). PFCC is the concept that patients/residents/clients and their family members are at the centre of health care. Health-care providers need to respect and empower persons and families to be genuine partners in their health. 

More than 45 LTC homes participated in the spring 2022 series of PFCC CoP to meet the new legislation. The CoP emphasizes learning from each other and ways to improve resident and family participation in care. Due to popular demand, RNAO is delivering the PFCC CoP again. 

This virtual offering will be a series of four one-hour facilitated webinars with additional opportunity for coaching by members of RNAO's LTC best practices team. This coaching will be provided during scheduled office hours outside of the webinar sessions. Don't miss out on this powerful opportunity to bring positive change and innovation to your workplace!

Participants must attend all sessions. Sessions will happen from 2 - 3 p.m. on May 31, June 28, July 26 and Aug. 23. 

  • session 1, May 31, 2-3 p.m. ET - Assessing current practices
  • session 2, June 28, 2-3 p.m. ET - Planning for Implementation
  • session 3, July 26, 2-3 p.m. ET - Implementing Practice Change
  • session 4, Aug. 23, 2-3 p.m. ET - Transforming your home & sustaining change 

The CoP includes:

  • facilitated interactive workshops 
  • working together with LTC homes across the province 
  • coaching by RNAO LTC Best Practice Coordinator to: 
    • assess participating LTC home’s current PFCC policies, procedures and practices using a gap analysis
    • develop and implement a PFCC action plan 
    • plan for ongoing evaluation of PFCC activities.
