Now available: new edition of Transitions in Care and Services best practice guideline
At RNAO’s 98th Annual General Meeting, the association released the second edition of its foundational guideline, Transitions in Care and Services. This guideline is central to the work of Best Practice Spotlight Organizations® and Ontario Health Teams (OHT). It includes evidence-based recommendations for nurses and the interprofessional team, organizations, and the health system overall, inclusive of OHTs.
The Transitions in Care and Services, Second Edition BPG is intended for nurses and members of the interprofessional team in many sectors including, primary care, acute care, home and community care, long-term care, rehabilitation, correctional facilities, shelters, and mental health and substance use health settings, to support persons and their networks during transitions in care.
This guideline was developed in collaboration with an interprofessional expert panel that included people with lived experience. RNAO extends a special thanks to the expert panel and co-chairs Shirlee Sharkey, former president and chief executive officer of SE Health, and Rhonda Crocker Ellacott, president and chief executive officer of Thunder Bay Regional Health Sciences Centre and chief executive officer of Thunder Bay Regional Health Research Institute.
