Join us for an engaging webinar showcasing the innovative and collaborative BPSO Carnival at H.J. McFarland Memorial Home, where best practice guidelines (BPG) are brought to life in everyday care. This event highlights how residents, families, staff and community members came together to explore evidence-informed approaches that enhance the resident experience.
Participants will learn about key initiatives, including the integration of Transitions in Care through the BOOMR program, which supports proactive medication reconciliation by connecting physicians, pharmacists, and residents and families prior to admission, helping ensure safer, more seamless transitions. The webinar will also highlight how participant feedback, gathered through admission surveys, informs ongoing improvements.
In addition, the session will explore advances in palliative and end-of-life care, where staff use RNAO assessments within PCC, such as the Palliative Performance Scale (PPS) to identify needs early and guide care planning. Attendees will hear how meaningful goals-of-care conversations are initiated at admission, supported by tools like the End-of-Life Wishes Booklet, and strengthened by family feedback collected through post-bereavement surveys.
This webinar will provide practical insights into successful BPG implementation, lessons learned, and strategies to support early identification, meaningful engagement with residents and families, and continuous quality improvement. Attendees will leave with actionable ideas and tools to strengthen care transitions, enhance palliative approaches, and drive positive outcomes in their own organizations.
