Interdisciplinary team strategies to prevent falls and injury at Nithview Home
After a difficult year marked by staff shortages, leadership changes, and a spike in resident falls, Nithview Home rebuilt its entire falls prevention program using RNAO best practices and strong interdisciplinary teamwork.
With support from their LTC Best Practice Coordinator, the home identified gaps, strengthened assessments, introduced post-fall huddles, launched comfort rounds, and equipped units with accessible fall-prevention tools. Staff education and family involvement also played key roles.
As a result, falls dropped significantly—from 61 early in the year to 34 by October—and the home is now sustaining its progress through ongoing monitoring and quality improvement.
