End of Life BPG: Champions Leading Practice Change

This presentation will provide an overview of how a team of champion leaders facilitated the successful implementation and sustainability of the End of Life Care in the Last Days and Hours Best Practice Guideline (2011) in a long-term care home.

During this presentation, participants will learn how to:

  • Outline the training, recruitment strategies and supports provided to develop a steering committee of champion leaders who are responsible for implementing the End of Life Care BPG in a long-term care home
  • Describe the strategies used by champion leaders to successfully engage front-line staff in BPG planning and  implementation across the organization
  • Describe the creative strategies used by the champion leaders and front-line staff to contextualize the End of Life Care BPG recommendations to their clinical setting
  • Discuss the steering committee's strategies for sustaining the work done with the implementation of the End of Life Care BPG in their organization
