RNAO will soon release the second edition best practice guideline (BPG) Transitions in Care and Services, a foundational guideline for all health sectors, and central to the work of Best Practice Spotlight Organizations® and Ontario Health Teams.



It includes evidence-based recommendations for: nurses and the interprofessional team, organizations and the health system overall, inclusive of Ontario Health Teams.



Join us on Tuesday, May 9, from 1-2 p.m. ET to learn about the creation of the guideline and its upcoming publication. During this presentation, we will describe:

  • the purpose and scope 
  • a sneak peek of key recommendation areas covered in the guideline 
  • how the guideline was developed and co-created with Best Practice Spotlight Organization® Ontario Health Teams (BPSO OHT)
  • how implementation of this guideline will support BPSOs and BPSO OHTs in the co-design of coordinated services that are person- and family-centred
  • how to begin continuously monitoring and evaluating the impact of this BPG
