The Best Practice Spotlight Organization (BPSO) initiative at St. Joseph's Care Group (SJCG) is focused on implementing best practice guidelines (BPG) in Transitions in Care and Services across the organization. What began as a nursing-focused initiative within a rehabilitation hospital is now expanding to all 11 of SJCG’s sites, including mental health inpatient and outpatient services, long-term care, physical rehabilitative and chronic care inpatient, and various outpatient programs.
This webinar will provide participants with a comprehensive understanding of how these BPGs can be applied across diverse healthcare settings to improve patient care. By learning to integrate evidence-based practices into their daily work, participants will help improve transitions in care and uphold professional standards across the organization.
An interdisciplinary approach, involving 12 key change partner groups such as N’doo’owe Binesi Indigenous Health team, professional practice leads, and quality practice councils, has been central to the selection of these guidelines. The webinar will guide participants through the process of collaborating across disciplines and organizational levels to ensure the success of BPG adaptation.
In summary, this webinar will empower participants to drive meaningful change within their organization, fostering an interdisciplinary approach to BPSO, and planning to implement across multi-sectoral organization.
