Moving into a long-term care home can be a very overwhelming process for the resident and the people in their support network. 

By implementing an admission process that is person and family centred, the home can capture a comprehensive, holistic picture of who the new admission is prior to their arrival. This promotes a smooth transition for everyone involved and allows the home to be proactive in anticipating the person’s needs and preferences from day one. This admission process creates a strong foundation for the resident and their care team to build on throughout their journey in the long-term care home.
