About this event:
RNAO is delighted to release the second edition of the Transitions in Care and Services best practice guideline (BPG), a foundational guideline for all health sectors, that is central to the work of Best Practice Spotlight Organizations ® (BPSO®) and Ontario Health Teams (OHT). This guideline includes evidence-based recommendations for:
- nurses and the interprofessional team
- organizations
- the health system, inclusive of OHTs
Join us Thursday, July 20, 2023 from 12-2 p.m. ET to celebrate the official launch of this much anticipated BPG. During this interactive session, participants can learn about:
- the purpose, scope and key recommendations areas addressed in this guideline
- the guideline development process, including how the guideline was developed by an expert panel of interprofessional practitioners, researchers, policy-makers and people with lived experience, in consultation with BPSO OHTs
- next steps for kickstarting implementation and evaluation of this BPG in your practice setting
Hear from panel members who played a crucial role in the development of this guideline, and health providers and people with lived experience who understand firsthand the importance of implementing best practices to improve transitions in care.
