About this event:

The Ministry of Long-Term Care (MLTC) is launching a modernized Health Assessment Form. This form is used to complete the health assessment required under the Fixing Long-Term Care Act, 2021 when a person applies for long-term care.

The form has been updated to clarify some questions, make the form easier to complete and better address the information needs of all partners who use the form.
