This webinar will explore how St. Francis Memorial Hospital successfully implemented the Confusion Assessment Method (CAM) as a standardized delirium screening practice tool on admission. In response to inconsistent screening practices that limited early detection, particularly among older adults and medically complex patients, the organization introduced a structured, evidence-based approach to strengthen nursing practice and improve patient safety. 

Participants will learn how the CAM tool was integrated into routine workflows through a thoughtful change management process that prioritized frontline nurse engagement, phased implementation and practical supports such as education sessions, unit huddles and just-in-time reminders. Ongoing leadership presence, auditing, and feedback reinforced accountability and normalized CAM completion as a core nursing responsibility rather than a temporary initiative. Alignment with best practice guidelines for dementia, depression, and delirium supported professional accountability, consistency, and accreditation expectations.

The session will also highlight the impact of standardized delirium screening on patient care, including early identification and intervention to reduce risks such as falls, prolonged length of stay, functional decline, and distress for patients and families. Emphasis will be placed on how this initiative strengthened people-centred care by promoting proactive communication, enhancing patient advocacy, and tailoring support for vulnerable populations. Engagement of the Patient and Family Advisory Council further ensured patient and caregiver perspectives informed education and implementation.

Overall, the webinar will demonstrate how sustained leadership, collaborative implementation and evidence-informed tools can translate into improved patient outcomes, enhanced safety, and a stronger culture of quality and continuous improvement.
