About this Event:

This webinar is intended for long-term care (LTC) leaders and direct care staff to learn how to access and use the resources/tools in the person-and family-centred care section in the LTC Toolkit, 2nd edition.

This session will focus on assisting LTC staff working in partnership with residents, and their families to make decisions about health care and services. The toolkit can be used by the Interprofessional health-care team to enhance the quality of their partnerships with residents, ultimately improving clinical outcomes and the experiences through the use of evidence-based practices.

Session Objectives
During this course, participants will learn how to:

  • navigate through the LTC toolkit to access the person-and family-centred care topic
  • use resources/tools for integrating evidence-based strategies for enhancing the health care experience
  • use the evidence-based resources/tools to support program planning, implementation and evaluation
