About this Guideline:

The Registered Nurses’ Association of Ontario in consultation with stakeholders, are developing a best practice guideline on coordination of care at transition points.

Coordination of care is essential for persons with complex care needs.

It is essential that nurses are aware of current best practices to support clients through transitions in care and in any environment and across all health-care and community settings.

The purpose of this guideline is to assist the profession with understanding the best practice opportunities to minimize the risk to clients when transitions, including sending and receiving patient/clients occur.
